Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:

  • Community Health Services : WIC, Commodity Supplemental Food, Foster Grandparents, Head Start, and Child Development Center.
  • Clinical Health Services : Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.
  • Supportive Health Services : Family Stabilization, a Teen Outreach Program, SOAR, Youth Leadership Program, Youth Shelter and Housing Assistance Programs.

Full-Time, Local Travel, No Weekends, Bilingual (Spanish) Incentive Offered

Remote (Binkelman, NE area only): While this is a remote position, we are only considering candidates who currently reside in or near Binkelman, Nebraska.

Areas of Responsibility: Generally working from a home office, promote access of migrant and seasonal farm workers to Migrant Health services and assist with clinic flow.

Major Duties:

  1. Positively promote the Migrant Health Program and facilitate accessibility for migrant and seasonal farm workers which may include transportation, to provide interpretation and translation services for medical, dental and pharmaceutical services.
  2. Work cooperatively with other agencies, programs and become knowledgeable regarding other services provided in the community and surrounding areas while acting as a resource for patients.
  3. Assist patients with completing all forms and applications in person, via phone or email.
  4. Conduct outreach work 2-3 days per week. Going to surrounding businesses, working with patients in person either in their homes or meeting them in a public place to assist with completing applications and retrieving documents such as proof of income (POI) or other forms. Work with other organizations in events to reach migrant or seasonal workers and their families.
  5. Schedule, cancel or reschedule appointments with providers, call in medication refills, new prescriptions and make referrals to other services.
  6. Follow up on missed appointments if needed.
  7. Return calls, emails, text messages and voicemails within 24-48 hours.
  8. Provide case management to chronic patients.
  9. Provide health education related programs and materials to patients.
  10. Knowledge of Jeenie, a language line accessed via phone or computer, or Google Translate for the cellphone or computer to assist with text messages or emails.
  11. Accurately enter data in Microsoft Word and Excel to update spreadsheets, keep patient’s information current and up-to-date in their charts such as patient current name, date of birth, current phone numbers, current mailing/physical/email addresses.
  12. Entering new and re-enrolled patient’s information into Practice Management (PM).
  13. Always maintain client confidentiality.
  14. Provide general clerical duties input data, scanning documents, answering and returning calls, texts, messages or emails in a timely manner, laminating, mailing, emailing applications or other forms. Following up with patients answering questions or concerns.
  15. Maintenance and care of company vehicle which may be provided. This vehicle will be strictly used for Migrant Health purposes only. Care includes interior and exterior cleanliness. Report any needed maintenance or damage to vehicle.
  16. Track mileage on the CAPWN milage form and turned in every Friday.
  17. Responsible for safely managing company credit card for fuel purchases.
  18. Credit cards may be issued for gasoline or other uses with prior approval from the Care Coordination Manager. Credit Card Request Form will need to be filled out and emailed to the Care Coordination Manager for approval prior to the card being used.

Working Conditions:

  • 50% home office environment with clients and with the public
  • 30% outreach in the community and clients
  • 20% training, meetings, and community work with other organizations.
  • Work requires meeting tight deadlines.
  • Significant travel in and around the region transporting clients and family members.

Physical Requirements:

  • Requires long periods sitting, driving and computer data entry.
  • Fine manipulation of hands with repetitive action to operate office equipment.
  • Occasional lifting and carrying up to 20 pounds.
  • Must regularly talk, hear, and have close vision ability.

Minimum Qualifications:

Grade: 15

  1. High school diploma or GED.
  2. Must be Proficient in communicating in Spanish and English.
  3. Age 21 or older.
  4. Must have a valid driver’s license with good driving record and insured vehicle (a company vehicle may be provided.)
  1. Physically and mentally able to perform duties.
  2. Available to work irregular hours and weekends.

Grade: 16

  1. Meet the minimum qualifications above;
  2. Proficient in communicating, writing and reading in Spanish and English.
  3. Three years’ experience in outreach and client referral or related area.
Grade 17
  1. Meet the minimum qualifications above;
  2. AA degree in Human Services, Social Work, or related field and one year experience in a human service program or five years’ experience in in outreach and client referral or related area.
