Department Description The APP is responsible for management of outpatient kidney transplant patients with the multidisciplinary team. Provides direct patient care, evaluation and medical management, development of assessment and plans for patients in outpatient kidney transplant clinic. Completes the documentation and serves as the primary provider for patients seen via telemedicine, in person clinic, and outreach clinics. Receives phone calls from patients and patient families and triages and escalates patient care alongside the physician staff. Participates in discharge planning and care coordination for transition of patient from inpatient to outpatient care. Completes lab review, medication management, immunosuppression management, and medical assessment of outpatient kidney transplant patients in person or over the phone. The APP is responsible for continuity of care and interfacing with other service lines for consultation when appropriate. The APP is responsible for assisting in transition of care from outpatient to inpatient hospital admissions for kidney transplant patients and working with the inpatient Transplant Nephrology team to transition patient care from inpatient to outpatient. The APP works with other members of the multidisciplinary team including Physicians, Fellow, Residents, Social Work, Case Management, Nursing Staff, Pharmacy, Dietician, and other service line providers. Assists with transition of care from inpatient to outpatient provider teams at time of discharge. The APP is expected to participate in patient care conferences, pathology conferences, radiology conferences, and departmental education as well as selection meetings. Works collaboratively with other APPs on kidney transplant service to provide patient care coverage. Actively communicates with other APPs on the kidney transplant service, attending staff, support staff and resident/fellow staff. Provides education regarding kidney transplantation to nursing staff, students, residents. JOB SUMMARY This paragraph summarizes the general nature, level and purpose of the job. Nurse Practitioner (NP): The Nurse Practitioner functions under the general Nurse Practitioner Standardized Process Protocol approved by the Interdisciplinary Practice Committee (IDPC). The Nurse Practitioner provides routine care and management of the patient with acute and chronic complaints, in collaboration with the supervising physician and through implementation of standardized procedures. Specific functions pertaining to the Service, Clinic or Department are established by the Nurse Practitioner and the supervising physician(s), and approved by the appropriate medical and nursing administrators, the IDPC, and the Stanford Hospital and Clinics (SHC) Medical Executive Board and Board of Directors. Physician Assistant (PA): The Physician Assistant functions under the general Physician Assistant Role Description approved by the Interdisciplinary Practice Committee (IDPC). Specific functions pertaining to the Service, Clinic or Department are established by the Physician Assistant and his or her supervising physician(s), and approved by the appropriate medical and nursing administrators, the IDPC, and the Stanford Hospital and Clinics (SHC) Medical Board and Board of Directors.
